    Graded Epidural Anaesthesia for Lower Segment Cesarean in a Parturient with Very Low Ejection Fraction

    ABSTRACT Dilated cardiomyopathy is characterized by left or biventricular dilatation with impaired ventricular contractility. It is associated with systolic dysfunction with decreased left ventricular ejection fraction and congestive progressive heart failure. Anaesthetic management of such condition is very challenging and requires highest level of expertise, strategy and precaution. We present such a case of dilated cardiomyopathy with a very low ejection fraction of 12% who underwent lower segment cesarean section under epidural anaesthesia with successful outcome.     Is Iron Deficiency Anaemia, a risk Factor For Wheeze Associated Respiratory Tract Infection in Children?

    Abstract: Introduction: Wheezing is the common in children with cumulative prevalence of almost 50% by the age of 6 yrs. Iron deficiency anaemia is an independent risk factor for lower respiratory tract infections (LRTI), however may also play an important role in wheeze associated respiratory tract infection. The present study aimed at finding a correlation between iron deficiency anaemia and wheeze associated respiratory tract infection. Materials and Methods: This case control study was conducted at a multidisciplinary tertiary care hospital from Jul 2012 to Jun 2013 among children six months to five years and having ≥2 episodes of wheeze associated respiratory tract infections. The clinical presentation and laboratory profile were recorded on a predesigned performa. Iron deficiency anaemia was diagnosed by haemoglobin <11.0 gm%, serum ferritin<12ng/ml and RDW>15%. The statistical analysis was done using SPSS, Version 15.0. Results: A total of 100 children were enrolled in the study, of which 50 cases and 50 matched controls. Majority of subjects in both groups were males (n=31;62%),with male to female ratio of 1.6:1. Proportion of cases with iron deficiency anaemia were higher among cases(32%) as compared to controls(24%) but the difference between two groups was not significant statistically (p=0.373) despite having higher risk(OR=1.49;95% CI=0.62-3.59). Mean monocyte, oeosinophil and absolute oeosinophil count was found to be significantly higher in cases as compared to controls (p<0.05). Conclusion: The present study do not suggest a possible linkage between iron deficiency anemia and wheeze associated respiratory infection, however its role as causative factor needs to be explored through long-term prospective and intervention studies on a large population.

    Magic of Intravitreal Triamcinolone and Dexamethasone in a Patient with Uveitic Macular Edema with Treated Ocular Tuberculosis

    ABSTRACT A 40-year-old woman with uveitic macular edema (UME) in a treated case of ocular tuberculosis (TB) is the subject of our unusual case study. She had full remission with a single intravitreal injection of triamcinolone and dexamethasone. Anti- inflammatory drugs, topical steroids, and anti-tubercular therapy were used to address the patient’s history of recurrent anterior and intermediate uveitis in past. When intravitreal corticosteroids were administered, the big macular cysts and related retinal thickness that were seen on the optical coherence tomography (OCT) results disappeared.     Validating breast self examination as screening modalities for breast cancer in eastern region of Nepal: A population based study

    Abstract Breast cancer is one of the most common causes of death in many developed countries amongst middle-aged women and is becoming common in developing countries as well. Aims and Objectives: The objective of this study was to evaluate the performance of Breast Self Examination (BSE) against examination conducted by the trained health personnel. Methodology: A descriptive evaluative study was conducted among women aged 15 to 60 years undertaken in urban and rural areas. It was a population-based study and non-probability sampling method was used for data collection. Respondents conducted Breast Self Examination on their own, and then investigators did clinical examination of breasts of the respondents to verify their findings. Results: Mean age of the respondents was 34.54 ± 9.12 years. Nearly half (44.8%) of the respondents were illiterate. On comparison of each variable of breast self- examination with the physical examination of breast, it was observed that size of the breasts and lump found in the breast had significant relation to performance during examination (P<0.1). The result of Kappa test showed 68% agreement between findings of examinations done by the experts and respondents. Conclusion: Breast Self Examination can be used as an important tool for primary prevention of breast cancer in Nepal, where sophisticated method like screening mammography for general public cannot afford.
