A laportomy had to be performed subsequently due to distension of the abdomen with air under the right hemi-diaphragm. Specimens of necrotic patches of the ileum revealed polyarteritis nodosa. The differential diagnosis and treatment in Nepal are discussed. This case report emphasizes the need to closely monitor patients with organophosphate poisoning so that the intermediate syndrome, a well known, life threatening phenomenon related to this poisoning, can be effectively dealt with.
