Publication Congenital Melanocytic Nevus and Fibroepithelioma of Pinkus: A Rare Type of Collision Tumor(Society of Dermatologists, Venereologists and Leprologists of Nepal (SODVELON), 2021) Bhatta, Anil Kumar; Keyal, Uma; Liu, YeqiangAbstract: Congenital melanocytic nevus is benign pigmented lesion of the skin usually present at birth. Here we report a case of 61-year-old male who presented with a pigmented lesion on the trunk, which on biopsy showed features of both congenital melanocytic nevus and fibroepithelioma of Pinkus. We reviewed literature for this combination and found only one case reported so far. Hence, it seems that it is very unusual to seefibroepithelioma of Pinkus in conjunction with a melanocytic lesion. Normally, T cells help body's immune system to fight off infection but, in CTCL, these cells develop abnormalities that make them attack the skin. It comprises heterogeneous group of skin neoplasms, the most common being mycosis fungoides (MF). Skin lesions are classified into patches, plaques, and tumor stage. Extra skin manifestations like enlarged lymph nodes are usually seen in plaques or tumor stage, when there are thick lesions on skin. It is usually a slow growing cancer that develops over many years. Herein, we present an 83-year female patient who suddenly developed extremely itchy patches on abdomen, which had spread to involve the entire trunk and all four limbs in about 10 days’ time. She already had lymphadenopathy and splenomegaly at the time of presentation. Skin biopsy and immunohistochemistry revealed CD4+ T cells CTCL. Moreover, bone marrow was hypercellular for age with 12% atypical lymphocytes. It is very unusual for CTCL to involve bone marrow at patch stage with no any plaques or nodules on the skin. Also, the sudden onset of skin lesions and its rapid progression with probably, the involvement of lymph nodes and spleen warrant further studies to guide diagnostic approaches and treatment recommendations for CTCLPublication Upcoming Therapeutic Modalities for the Treatment of Keloids: An Update(Society of Dermatologists, Venereologists and Leprologists of Nepal (SODVELON), 2019) Keyal, Uma; Bhatta, Anil KumarAbstract Introduction: Keloids are benign dermal tumors that generally form by local fibroblast proliferation and excessive collagen production following skin trauma. Several methods have been described for the treatment of keloid, often with suboptimal results and recurrences. Objective: To update literature that provides information regarding upcoming therapeutic modalities for the treatment of keloids, including lasers, radiofrequency (RF), photodynamic therapy (PDT), and ultraviolet A1 (UVA1) irradiation. Materials and Methods : A systematic review of the literature was performed for the original articles related to the treatment of keloids. The search terms ‘keloids’ and ‘lasers’ or ‘radiofrequency’ or ‘PDT’ or ‘UVA1’ was entered into a search of the National Library of Medicine’s PubMed Database. Keywords: Collagen, Fibroblasts, Keloid, Photochemotherapy, Ultraviolet therapy Result: The search returned a total of 188 sources, of which, 26 articles met our inclusion criteria. Conclusion: Combination approach is superior than solo therapy in the treatment of keloid. However, it is highly desirable that new emerging therapies undergo large-scale studies with long-term follow-up before being recommended conclusively as alternative therapies for the treatment of keloid. Moreover, lack of randomized clinical trials (RCTs) needs to be taken into consideration urgently.
