Publication Agenesis of Dorsal Pancreas in a Young Adult: A Case Report(Nepal Medical Association, 2023) Ulak, Nistha; Bharati, Manjil; Bhattarai, Suju; Ranjit, Philip Shyam; Byanju, RabinaAbstract Dorsal agenesis of the pancreas is an exceedingly rare congenital anomaly where the dorsal part of the pancreas fails to develop properly during embryonic development. We report a case of partial agenesis of the dorsal pancreas in a 27-year-old female who presented with progressively worsening abdominal pain, nausea, vomiting, generalized weakness, easy fatigability, and dizziness. Physical examination revealed fair general condition with stable vital signs and normal abdominal and other system findings. Laboratory evaluations and abdominal computed tomography scan revealed an absence of the pancreatic body, tail, and duct of Santorini, confirming the diagnosis of partial agenesis of the dorsal pancreas. Only around 100 cases have been reported in the literature, making it a diagnostic challenge. Our case illustrates the rarity and challenges in diagnosing dorsal agenesis of the pancreas. Further research is needed to fully understand its causes and associations.Publication Massive Middle Cerebral Artery Ischemic Stroke Associated with Severe Acute Respiratory Distress Syndrome in H1N1 Influenza: A Case Report Authors(Nepal Medical Association, 2022) Bhattarai, Suju; Karki, Bipin; Pokhrel, Shweta; Regmi, Sachit; Shrestha, Bibek; Shrestha, Pramesh SunderAbstract: Influenza has a common occurrence during its peak seasons. It usually causes disease of the respiratory tract including severe acute respiratory distress syndrome. However, it may also cause disease and complication of other organ systems. We present a rare complication of influenza in which a patient secondary to influenza developed massive middle cerebral artery ischemic stroke. The patient however survived following recovery of both severe acute respiratory distress syndrome and ischemic stroke after decompressive craniectomy and a prolonged intensive care unit stay. This case report is to highlight the importance of influenza related complications besides the pulmonary infliction which can lead to morbidity and even mortality if not managed on time.Publication Systemic Sclerosis with Inflammatory Myositis: A Case Report(Nepal Medical Association, 2023) Bhattarai, Suju; Pudasaini, Jyoti; Sharma, Ashutosh; Bharati, Manjil; Dhakal, NarayanAbstract The overlap of systemic sclerosis and inflammatory myositis is a rare disorder which results in immune system activation and production of autoantibodies. We present a case of a 28-year-old female with complaints of generalized weakness, multiple joint pain, facial puffiness, and blackish discolouration of skin over the last 4 months. Blood investigations demonstrated autoantibodies positive for overlap syndrome. She was managed with hydroxychloroquine, mycophenolate mofetil and steroids. This case report highlights the importance of early diagnosis and treatment of this rare entity for better patient outcomes.
