Publication A Scenario of Poisoning in Children in Manipal Teaching Hospital(Nepal Paediatric Society (JNPS), 2011) Malla, Tejesh; Malla, Kalpana K; Rao, KS; Gauchan, Eva; Basnet, Sahisnuta; Koirala, DPAbstract: Objective: To see the pattern and prevalence of poisoning in children in Manipal Teaching Hospital, Pokhara. The cases were also considered in relevance to the age groups with various agents, the commonly observed clinical features in various poisonings and mortality. Study design: Hospital based retrospective study. Study period: 4 years (January 2006 – January 2010). Study place: Department of Paediatric, Manipal Teaching Hospital, Pokhara. Material and Methods: A detailed study of all the case files of the children admitted with various acute poisonings, where the causative agent was known, was done. Results: A total number of 94 children (56 males & 38 females) were admitted with poisoning (1.79% of the total admissions) during that period. Maximum number of children were of the preschool age group i.e. < 5 years (64.89%). Organophosphorus Compounds (OPC) (27%) and Kerosene Oil (23%) were the two most frequent agents involved. The other agents included Snake bite (19%), Drugs/ Chemicals (16%), Dhatura (12%), and Mushroom poisoning (3%). The most common nature of poisoning noted was accidental (95%). The mortality rate observed was 6.38%. Conclusion: OPC and Kerosene Oil poisoning were the two most common poisoning observed in this study comprising almost half the cases; accidental poisoning was the most common pattern noted and there was a male predominance. Early recognition and timely treatment can decrease the mortality.Publication Doss Porphyria (δ-Aminolevulinic Acid Dehydratase Porphyria) Presenting with Acute Onset Flaccid Paralysis(Nepal Paediatric Society (JNPS), 2015) Singh, Sandip Kumar; Gauchan, Eva; Koirala, Deepak Prasad; Rao, KSAbstract: δ–Aminolevulinic acid dehydratase porphyria is an autosomal recessive disorder of heme synthesis resulting from deficiency of δ-aminolevulinic acid dehydratase (ALAD). Patients present with fatal neurovisceral manifestations and motor neuropathy. Here we report a patient with rapidly progressive flaccid tetraplegia with respiratory and bulbar paralysis. The importance of early diagnosis, prompt treatment and screening of relatives is stressed.Publication Ellis Van-Creveld Syndrome in an Ethnic Group in Nepal: Case Report with Review of Literature(Nepal Paediatric Society (JNPS), 2016) Gauchan, Eva; Sharma, PrakashAbstract: Ellis van-Creveld syndrome is a rare autosomal recessive disorder characterized by acromesomelic dwarfism, postaxial polydactyly, oro-dental abnormalities and congenital heart disease. We are reporting three cases in two families of the same ethnic group from Western Nepal. This is a case report of a neonate delivered to Manipal Teaching Hospital, Pokhara with features suggestive of VACTERL association.Publication High Red Cell Distribution Width as a Biomarker of Mortality in Critically Ill Paediatric Patients(Nepal Paediatric Society (JNPS), 2020) Gauchan, Eva; Basnet, SahisnutaAbstract: Introduction: Red cell distribution width (RDW) is a frequently overlooked parameter in routine haematological reports. It is a simple and inexpensive test which has been found by many adult studies to be a prognostic indicator of mortality in intensive care units. The objective of this study was to see if high RDW could be used as a marker to predict mortality in critically ill children. Methods: This was a prospective observational study conducted in the paediatric intensive care unit (PICU) of a tertiary hospital of Western Nepal over a period of one year. Study subjects were selected by purposive sampling method. RDW at admission and relative change in RDW (ΔRDW) was compared to see if they had any role in predicting mortality in this group of children. Receiver operating curve analysis was plotted to find an optimal cut-off point to define high and low RDW and various outcome parameters were analysed. Results: Out of 131 children, there were 12 (9.1%) mortalities. Admission RDW was higher in the death group as compared to the survivor group (17 vs 14.6; p = 0.012). Similar finding was seen with ΔRDW (0.45 vs 0.00; p = 0.006). ΔRDW above the cut-off value of 0.15 was found to be associated with a generally more complicated course during hospitalisation as well as had more risk of mortality. Conclusions: Both RDW and ΔRDW above the cut-off value were found to be associated with mortality. In addition, high ΔRDW was also found to predict a more complicated course during hospitalisation.
