    Clinical observations among patients with Dengue Fever in Nepal

    Abstract Introduction: Dengue is an arthropod-borne viral illness that has become a serious public health threat in Nepal. The disease has been reported frequently during the post monsoon period in the southem lowlands of the country. Studies relating to sero-prevalence of dengue have been performed regularly in Nepal but the most frequent clinical observations among the patients with dengue fever have not been well documented. We report thrombocytopenia (74%), headache (44%) and myalgia (44%) among the febrile cases as the most frequent features of the 287 dengue cases identified by rapid immunochromatographic tests and ELISA during 2010 and 2011. These findings are potentially helpful for the clinicians to predict dengue in the early stage in those areas where dengue has been endemic, thus minimizing the significant morbidity and mortality of the disease.     Comparison of Axillary Temperature with Oral Temperature and Determination of Optimum Placement Time of Thermometer in Adults of Teaching Hospital, Nepal

    Abstract Introduction: Accurate temperature measurement is important to person, in whom suspicion of infection could result in investigations, administration of antibiotics, and even hospital admission. This study was done with the aim to determine the optimal dwell time of the thermometer in oral and axillary sites required to accurately reflect the stabilized core body temperature. Method: Cross-sectional survey of 113 adult patients was done who were admitted in Manmohan Cardio- Thoracic Vascular and Transplant Center (MCTVC) in the period between 16thJuly to August2013. Ethical approval from the Institutional Review Board (IRB) of TUIOM and permission from the respective hospital authority was taken before data collection. Conventional glass mercury thermometer were checked and brought to 95 degree of Fahrenheit before each measurement of the temperature. Two thermometers were simultaneously placed in oral cavity and in the axillary skin sites. Reading of the oral temperature was done in 1 and 3 minutes, similarly, reading of axillary temperature was done in 3 and 5 minutes. Data was analyzed in SPSS 18 by using parametric and nonparametric tests. Results: Findings revealed the mean temperature difference between 1 minute oral to 3 minute axilla was 0.92 ± 0.79 SD, and the 3 minute oral to 5 minute axilla was 0.96 ± 0.82 SD, the difference was significant (P<0.05). Further, the mean oral temperature in 1 minute was 97.75 ± 1.27 SD, and in 3 minutes was 98.69 ± 1.37 SD. Similarly, mean temperature of axilla in 3 minutes was 97.67± 1.40 SD, whereas mean in 5 minutes was 98.43 ± 1.46 SD. The mean difference was significant. Conclusion: The effective time to measure oral temperature was three minute and it was five minutes for axillary measurement.     Knowledge and Attitude Regarding Pubertal Changes among Secondary Level Students of Selected Schools of Dhulikhel Municipality

    ABSTRACT Background Adolescence is a transition phase of life during which a lot of pubertal changes occur. If not dealt properly, many mental and behavioral disturbances can result. Adolescents must have good knowledge and attitude regarding physical and psychosocial pubertal changes. Objective To assess the knowledge and attitude regarding pubertal changes among secondary level students of selected school of Dhulikhel Municipality. Method A quantitative, descriptive cross-sectional study was conducted among secondary level students of Dhulikhel Municipality. Study was conducted among students of grade 9 and 10 of four government and three private schools of municipality. After selection of seven schools, proportionate stratified random sampling technique was done followed by lottery method to select required number of students. Both descriptive statistics and inferential statistics were applied using Statistical Package for Social Science (SPSS) Version 25.0 for data analysis. Result The mean score of knowledge regarding pubertal changes was 26.2±2.9. Likewise, the mean attitude score regarding pubertal changes was 55.4±1.2. Knowledge regarding pubertal change was significantly higher among age group 15-17 years. Females had higher knowledge compared to males. Attitude related to pubertal change was significantly more in respondents who studied in government school. Conclusion Adolescents of Dhulikhel municipality schools had good knowledge and attitude about pubertal changes. KEY WORDS Attitude, Knowledge, Pubertal change
