A Transitional Experience: Basic Science to Clinical Rotation
(Nepal Medical Association, 2023) Pradhan, Bishal; Rokaha, Prerana Singh; Lama, Ashmina
Abstract Bachelor of Medicine and Bachelor of Surgery is a roller coaster journey that is educational yet emotional. Because of how the context and responsibilities change throughout time, learning is exciting. Nevertheless, the exposure to basic science during this course teaches us to be diligent, committed, and compassionate, and it also gets us ready for the next stage of clinical practice. For us as students, the primary areas that change as a result of this transformation are professional networking, workload, patient interaction, time management, leadership, and communication skills. In this journey, the transition is inevitable so we need to adapt to change seamlessly. Additionally, extracurricular activities play a significant role in this journey.

Dandy-Walker Malformation with Neonatal Meningitis: A Case Report
(Nepal Medical Association, 2024) Pradhan, Bishal; Sharma, Bishal; Acharya, Pratistha; Thapa, Suraksha; Chand, Jyoti; Bista, Simran
Abstract Dandy-Walker syndrome is a rare congenital central nervous system malformation. Dandy-Walker variant is characterised by cerebellar vermian hypoplasia, cystic fourth ventricular dilatation, and normal posterior fossa volume. Various prenatal tests such as ultrasound, fetal magnetic resonance imaging, and amniocentesis can help diagnose Dandy-Walker syndrome. Here, we report a case of the Dandy-Walker variant with meningitis in a neonate admitted to the neonatal intensive care unit due to multiple petechiae on the anterior abdominal wall, accompanied by peripheral cyanosis at the time of birth. Although maximum cases are diagnosed prenatally, some cases might be missed due to inadequate antenatal examination. Magnetic imaging resonance of the brain is best for the diagnosis of Dandy-Walker syndrome postnatally.

Lipomeningocele with Tethered Cord Syndrome in an Adult: A Case Report
(Nepal Medical Association, 2023) Pahari, Soumya; Thapa, Suraksha; Bhattarai, Bibek; Pradhan, Bishal; Bhandari, Paawan Bahadur; Baniya, Purushottam
Abstract Lipomeningocele is a closed neural tube defect characterized by the presence of spinal tissue within the spinal cord, with a junction between the spinal cord and a lipoma. It has a prevalence of 0.6 per 10,000 live births. A 29-year-old female presented to the outpatient clinic with complaints of weakness in both legs (more on the right) and tingling sensation and pain over her right thigh and legs for 1 year. She also mentioned swelling in her lower back since birth, which has been growing progressively since childhood. Magnetic resonance imaging of the spine revealed a low-lying spinal cord with the conus lying at the lower end of the L2 vertebral body. Spinal decompression with detethering of the cord and excision of the lipomeningocele was undertaken via a posterior midline incision. Prompt surgical intervention are crucial for symptom relief and prevention of neurological deterioration.
