    Etiology and Demographic Profile of Medico-legal Cases in Emergency of a University Hospital in Nepal

    ABSTRACT Introduction: Any injury or ailment where investigations by the law-enforcing agencies are crucial are considered as medico-legal cases. The objective of this study was to describe the profile of medico-legal cases presenting to the Emergency Room (ER) of Tribhuvan University Teaching Hospital (TUTH), Kathmandu, Nepal. Methods: A retrospective review of medical records was done to include all medico-legal cases presented from July 2018 to June 2019. Results: Medico-legal cases comprised 1805 (3.8%) of all cases (47,289) in the study period. The category of 25- 39 years was the commonest age group involved [659 (36.5%)] followed by 15-24 years group [582 (32.2%)]. Males were almost twice (65.8%) in number compared to females (34.2%). The top three reasons for the ER visit were road traffic accidents (RTAs) [757(41.94%)], physical assaults [356 (19.72%)] and poisoning [239 (13.24%)]. Other causes were burns, falls, sexual assaults, hanging, injuries (gunshot, electrical and machinery) and drug overdose. Conclusion: There are significant numbers and wide varieties of medico-legal cases that present in the ER of TUTH. Young males are the most high-risk population and RTAs are the commonest etiology. This study will help the hospital in capacity strengthening of the ER, based on the volume of individual case types.     Knowledge and Self-Care Practice on Hypertension among Hypertensive Patients in a Tertiary Level Hospital of Kathmandu

    ABSTRACT Introduction: Hypertension is a public health challenge and major risk factor for cardiovascular disease in the developing as well as developed countries. The successful management of hypertension depends upon patient’s knowledge on hypertension and self-care practices. Methods: A descriptive cross-sectional study was conducted to identify the knowledge and self-care practice on hypertension among 212 hypertensive patients attending in outpatient department of Manmohan Cardiothoracic Vascular and Transplant Center (MCVTC). Non-probability purposive sampling technique was used. Data was collected through face-to-face interview by using structured questionnaire. Data was analyzed using SPSS version 16.0. Results: This study showed that 57.5% of the respondents had adequate level of knowledge on hypertension and 57.1% of the respondents had good overall self-care practice. Regarding self-care practice, we found 100% medicine adherence, 85.8% respondents avoided alcohol and 84.9% avoided smoking, 50.9% followed weight management practice, 48.6% followed dietary management and 44.8% performed physical activity. Level of knowledge was significantly associated with age, gender, educational status and occupation. Self-care practice was only significantly associated with educational status. Level of knowledge and self-care practice were significantly positively correlated. Conclusion: More than half of the respondents had adequate level of knowledge. Regarding self-care practice, nearly sixty percent of the respondents had good self-care practice. To bring those rates to higher level, awareness programs should be launched about diseases and self-care practice on hypertension. Keywords: Hypertension, knowledge, self-care practice

    Need of Specialized Courses in Advance Nursing Education in Nepal

    ABSTRACT Introduction: Nursing specialization and certification of skill contribute nursing professionalism that improve nursing practice in an identified specific area within the entire field of professional nursing. Methods: A descriptive cross sectional survey was used for assessing the need of specialized courses in advanced nursing. Data was collected from January to March 2022 through an online survey among 354 nurses working in Nepal. Collected data was entered into SPSS version 16 and expressed by using descriptive statistics. Results: The findings showed that more than half (54.5%) of the participants were age group of 20-29, 99.4% were females, 50.3% had bachelor’s degree, 60.7% worked in government and 70.3% had working experience less than 10 years. Almost all (98.8%) felt importance of specialized courses in higher education and the needs of advanced courses (72.9%) and especially in critical care (53.1%) Almost all respondents (95.2 to 90.7%) perceived these courses increase self-esteem and morale, promote quality patient care, increase confidentiality and creditability, ensure competency, validate nurses’ knowledge and expertise, advancement of professional career ladder and clinical expert and increase professional accountability. Majority of them perceived (77.4%-57.6%) that specialty courses increase access to job-related power, obtain noticeable financial benefits and perform excellence services. Almost all (91.0%) of them perceived that it is beneficial to maintain professionalism. Conclusion: This study concluded that almost all graduate nurses felt specialty courses in masters degree for personal and professional development. Thus it is strongly recommended to initiate different specialized courses at the postgraduate level. Keywords: Advance nursing education; need; specialized courses
