Publication Aspiration of a Row of Artificial Dentures in an Adult: A Case Report(Nepal Medical Association, 2022) Dulal, Malati; Tripathi, Prashant; Shrestha, Amit; Acharya, Kunjan; Niroula, SaileshAbstract: Clinical findings of foreign body aspiration, generally, are subtle. Scrutinous clinical suspicion is always recommended. Here, we present a rare case of an adult male, who under the influence of alcohol had aspirated a row of his artificial denture without his conscience of where his denture got missing and presented to our outpatient department with non-specific symptoms. With clinical examination and advanced diagnostics, he was successfully managed with rigid bronchoscopy. The objective of this study was to determine the diagnostic accuracy of fine needle aspiration cytology (FNAC) as compared to the histopathology in parotid gland swelling. Methods: It was a descriptive cross sectional study carried out in the Department of ENT-Head & Neck Surgery, Tribhuvan University Teaching Hospital, Institute of Medicine, Kathmandu by reviewing the medical record charts of the patients who had undergone surgery for parotid lesions during the study period of seven and half years. All patients whose fine needle aspiration cytology and histopathology reports were available were included in the study. The data were presented as mean, standard deviation, ratio and percentages. Microsoft excel was used for data analysis. Results: There were 75 patients included in the study. The age ranged from nine years to 78 years and the mean age being 38.3± 17.42 years. The male to female ratio was 1:1.78. The concordance rate between fine needle aspiration cytology and histopathology was 82.7%. The sensitivity and specificity of the fine needle aspiration cytology were 80% and 95% respectively. Similarly, the positive predictive and negative predictive values were 84% and 93% respectively. The diagnostic accuracy of the fine needle aspiration cytology was 91% for the parotid swelling in our study. Conclusions: The diagnostic accuracy of fine needle aspiration cytology for parotid swellings in our study was excellent. The result of fine needle aspiration cytology is helpful in deciding management plan for parotid lesions. Keywords: Benign; cytology; histopathology; malignant; parotid.Publication Effectiveness of Single Dose Oral Corticosteroids in Relief of Pain due to Acute Tonsillitis and/or Acute Pharyngitis(Institute of Medicine, Tribhuvan University, 2024) Rauniyar, Neeraj; Kumar Sah, Santosh; Tripathi, PrashantAbstract: Introduction Throat pain due to acute tonsillitis and acute pharyngitis hamper with oral intake, speech and swallowing leading to feeling of general weakness and inability to work. The objective of this study was to evaluate the efficacy of single dose of oral corticosteroids for improving or resolving of pain in adults with acute tonsillitis/acute pharyngitis. Methods This study was quasi-experimental study conducted on patients aged 16 years and above with acute tonsillitis/acute pharyngitis were included in the study. A total of 88 patients were included out of which, 46 patients received steroids as a single dose of oral prednisolone (Group A) and in 42 patients, steroids were not used (Group B). Pain score, assessed at 24 hour and 48 hours on Visual Analogue Scale (VAS); and pain duration were compared between two groups. Results There was a male preponderance with male to female ratio of 3:1. Age ranges from 17 to 32 year. The most common symptom was odynophagia presenting in 86 patients out of 88. Pain score at 24 hours was 4.59±0.98 hour in group A and 6.28±1.21 hour in group B (p<0.001). Pain score at 48 hours was 0.41±0.68 hour in group A and 2.98±0.90 hour in group B (p<0.001). Time until onset of pain relief was 3.56±0.98 hour in group A and 5.90±1.08 hour in group B (p<0.001). Time until complete pain relief was 42.61±6.44 hour in group A and 58.71±17.19 hour in group B (p<0.001). Conclusion In adult patients with acute sore throat due to acute tonsillitis and/or acute pharyngitis, single dose of oral prednisolone was effective in reducing pain as well as shortening the duration of pain when compared to the group where it was not used.Publication Old World Mucosal Leishmaniasis Treated with a Single Dose of Amphotericin: A Case Report(Institute of Medicine, 2024) Thapaliya, Sabin; Poudyal, Prasanta; Tripathi, Prashant; Thapa, Milan; Anjan, ShresthaABSTRACT Mucosal leishmaniasis, an uncommon disease in the old world, is recognized as an occasional complication of donovani related visceral leishmaniasis. It usually affects the spleen, liver, skin, mucosa of the respiratory tract and, less commonly, the oral cavity. Our case was a young adult with non-healing lesions in the oropharyngeal region who was diagnosed as mucosal leishmaniasis during evaluation for suspected mucosal carcinoma.
