    A case of leiomyosarcoma of the kidney

    Abstract Introduction: Sarcomas of the kidney are rare.Among primary sarcomas, leiomyosarcoma is the most frequent. A case of primary renal leiomyosarcoma is presented. Case report: This report concerns a 48-year-old woman, who presented with slowly increasing right flank mass and dull aching pain. A well-defined, gray, firm mass measuring 12.5 x 10.5 cm was histologically composed of interlacing bundles of spindle cells with cellular atypia and nuclear hyperchromasia. Mitotic figures constituted 10-12/ 10 HPF. Tumor cells showed positivity for desmin. The differentiation from sarcomatoid renal cell carcinoma is discussed. Conclusion: The case is reported for its rarity and this is the first reported case of renal leiomyosarcoma in Nepal.

    Local and systemic eosinophilia in patients with nasal polyps

    Abstract Background: To determine the presence of local and systemic eosinophilia in patients with nasal polyps and to correlate between them. Materials and Methods: Patients of all age group, presented in ENT OPD during October 2004 to May 2006, with ethmoidal polyps were studied. Patients using steroids in last one month, antrochoanal polyps and histopathological diagnosis other than nasal polyps were excluded. The local eosinophils count in nasal polyp tissue of study group and inferior turbinate mucosa of control group were evaluated and analyzed. Similarly absolute blood eosinophil counts in both study and control group were evaluated. The association between local and systemic eosinophilia were analysed. Results: Out of 78 patients with nasal polyps systemic eosinophilia was seen in 28 patients (35.9%) and local eosinophilia was seen in 50 (64.1%) patients. Among 30 patients of control group only 2 patients (6.7%) had systemic and none had local eosinophilia. On analyzing the results, the presence of systemic and local eosinophilia in patients with polyps were statistically significant with p value 0.005 and 0.0001 respectively. The association between them was also significant (p <0.025). Conclusion: There is significant local and systemic eosinophilia in patients with nasal polyps and also significant correlation between them.     Retinoblastoma in a 37 years old man in Nepal: A case report

    Abstract Retinoblastoma is extremely rare in adults. We report a case of Retinoblastoma diagnosed by histopathology in an enucleated specimen of a 37 year old patient who presented with pain and diminished visual acuity with intraocular mass and serous detachment. CT head and orbit showed uniform hyperdense mass in nasal quadrant of left globe projecting into adjacent vitreous cavity. Chest X Ray and USG abdomen was unremarkable. Initially provisional diagnosis of amelanotic melanoma was made. In view of nil visual prognoses enucleation was done. Sections from the enucleated eyeball showed diffuse proliferation of tumour cells. These tumour cells were small sized with scanty cytoplasm, and oval and hyper chromatic nuclei. A diagnosis of poorly differentiated Retinoblastoma was made with TNM (AJCC) stage as T1NXMX. This paper highlights the delayed presentation of retinoblastoma in adult. Although retinoblastoma is rare in adult, we suggest keeping retinoblastoma in differential diagnosis of intraocular mass until proven histopathology report is available.
